How to open your own "homemade" Windows Phone App with NFC tags?

Are there NFC applications , which you can open your own "homemade" applications with Windows Phone? I have searched all over the internet but I haven't found any yet. Does someone know where on the internet I can found this kind of nfc apps?

With this application you can't launch homemade apps created with Visual Studio. And I need just that kind of NFC application. I had the such application on my old Windows Phone device but I can't recall it's name. Unfortunately I don't have any more my old Windows Phone device. Does someone recall if such NFC application does exist?
